Quality Online Teaching Academy - Application

This is the application to participate in the 2017-18 Quality Online Teaching Academy. Deadline for applications is Friday, June 16. Applicants will be notified of their application status within a week of the deadline.

Please provide the brief designation for the course intended as the focus of your Academy work (e.g., BIO 160).
Please estimate the annual enrollment of all of your sections of this course that would be taught online.
For the course that you want to teach online, please explain why it is a strong candidate for conversion to an online format. You can include factors such as whether it is a curricular bottleneck, a high-demand, or a high-failure-rate course or other rationales supporting the value of an online version of the course. Include evidence of waiting lists, expanding future demand, etc.
Please summarize your background with online course design and instruction and share your interest in developing your knowledge and skills in this area. Please note: Experience is not required.
Please indicate your commitment to the full participation in the Academy experience 2018-19. Full participation includes: (1) Introduction to Online Teaching workshop (Summer 2018), (2) Online Teaching Learning Community (Academic Year 2018-19 - 10 sessions), (3) eLearning Addendum and QOLT Springboard workshops, (4) getting the course scheduled and teaching it, and (5) submitting the course for official QLT Rubric review.
Please request that your department chair/head and your college dean forward an email or memo attachment indicating that each has reviewed your proposal and (1) support your participation in the Academy, (2) support the course selected for online conversion, and (3) commit to scheduling the online course taught by you by Spring 2018. Email or memo should be directed to Catherine Hillman, QOTA Coordinator, at cehillma@calpoly.edu and Patrick O'Sullivan, CTLT director, at posulliv@calpoly.edu.
